The writing that shrinks, and the fingers that fumble

Parkinson's often reaches the hands in small, daily ways. Handwriting can start at a normal size and then tighten and shrink across a line or a page — a pattern called micrographia. Buttons, zips, cutlery, and coins can turn a quick task into a slow, effortful one. None of it is dramatic, and all of it can quietly wear on the ordinary business of a day.

The Steady Hand rehearses the opposite quality: a hand that moves fluidly, and writing that keeps its full size all the way along. Because imagined and real movement share circuitry — and because reduced movement amplitude is a core part of the problem — mentally rehearsing large, easy, flowing hand movements may help make a little more of that fullness available.

This is mental rehearsal — picture it, don't perform it. You'll be imagining the movements while your hands rest, not writing or manipulating objects during the practice. It's a rehearsal to do at rest, then carry into the real tasks separately.

Occupational therapy is the practical partner here. For hand function, handwriting, and daily tasks, an occupational therapist is the professional who can help most — with amplitude and handwriting strategies, adapted tools, and tailored exercises. Ask your GP or neurologist for a referral if fine-motor difficulties affect your day. This imagery practice sits alongside that work; it doesn't replace it, and it doesn't replace your medication, which often eases hand slowness as part of your dose cycle.

Settle comfortably, hands resting in your lap or by your sides. You'll be picturing the hand at work, not putting it to work — nothing to hold, nothing to write.

About the figure

Who the figure is — and isn't

In Phase I, a figure is seen writing and using their hands with easy, full-sized movements. As with every practice here, this figure is not you before Parkinson's, and not a former hand to mourn or chase back. There's no going back, and the practice never implies it.

The figure is only an embodiment of fluid, ample hand movement — shown so you can study the quality and then feel it from the inside. When the figure steps into you, you borrow the feeling of a hand that stays full and easy, and find it in your own hands as they are today.

·Opening — Breath and Light

And you're here now, settled, your hands resting and asked to do nothing at all. Let them be heavy, or light, or somewhere in between — however they happen to be is exactly right.

There's a little time set aside, and nothing to accomplish in it. Just the rehearsing of ease, gently, while the hands themselves rest.

You're breathing, easily and on its own. And you might let your attention drift, in a moment, down the arms and into the hands — those remarkable, expressive things that do so much of our reaching out into the world.

A soft warmth, settling into the palms and the fingers. Not a task, not a stretch — just warmth, and ease, resting in the hands like sun on a windowsill.

IPhase I — The Ideal Being

A little way off, there's a figure — sitting at a table, writing. You may see them clearly, or simply sense them there. And notice the hand. It moves fluidly, easily, the pen gliding along without gripping or cramping. And notice, especially, the size of the writing: full and open, each letter as generous as the last, staying that way all the way along the line. Nothing shrinking, nothing tightening. The hand simply keeps its fullness, effortlessly.

And now the figure sets down the pen and does something ordinary — fastens a button, perhaps, or picks up a cup. Notice how the fingers find their way without fuss: smooth, unhurried, sure. Not fast, not forced. Just easy, full movements that do what they mean to.

This is the quality to stay with: not speed, and not precision under pressure, but fullness and flow — the hand moving at its natural, ample size, relaxed and continuous. Writing that stays large. Fingers that stay easy.

And now the figure turns, and comes closer, and steps into you — gently — so that being near it becomes doing, from the inside.

Take in your own hand moving that way. An imagined pen, gliding — the letters full and open, keeping their size across the line. An imagined button, an imagined cup — the fingers easy and sure. Not forcing anything. Just rehearsing a hand that stays full and fluid. However your hands are today, you've taken in the ease of them, and that's yours to keep.

IIPhase II — The Working Hand

A great deal of the brain is given over to the hand — far more than its size would suggest. Movements of the fingers are shaped by the motor cortex, drawing on the same deep structures, the basal ganglia, that help keep any movement at its full size. In Parkinson's, when those structures run short of dopamine, movements tend to shrink — and the hand, which trades in small, precise movements, feels it keenly. That shrinking is the micrographia, the tightening grip.

Here's the useful part: movement size responds to intention and to attention. When you deliberately aim for large, full movements — in rehearsal, and then in practice with your therapist — you push back against the shrinking. Aiming big helps the movement land at a more normal size.

So bring the full, easy hand to mind once more — the writing keeping its size, the fingers moving amply and without cramp. Not straining. Just holding the intention of fullness, quietly, so the pattern of a generous movement stays warm and familiar, ready for the real page and the real button, and for the exercises your occupational therapist gives you.

And when you're ready, in your own time, let the room return, your hands resting where they are. No need to test anything, and no rush to reach for a pen.

Carry a little of it with you, however much came — the sense of a hand that stays full and easy, the intention of writing that keeps its size. It's a pattern you've rehearsed, and it will be here to rehearse again.

There was nothing to get back to here, and nothing to prove. This practice sits gently beside the real work of the hands — your occupational therapy, your own daily practice, your medication. It's a rehearsal of ease, offered to your hands as they are today. And whenever you'd like it, the full, easy hand is here to bring to mind again.

The pathway in Phase II

What the evidence says

Emerging — the lightest evidence of the motor practices

Where the "Emerging" badge comes from

Of the motor practices, hand function has the least evidence behind it, and the badge is honest about that. The relevant work is small and preliminary.

A handful of studies from one research group have shown that combining action observation and motor imagery can influence the amplitude of hand movements in Parkinson's — a plausible route to the reduced-size problems behind micrographia — and have developed a home-based observation-and-imagery programme for functional hand movements, designed together with people who have Parkinson's. These are encouraging early steps: experimental and feasibility work, small samples, not yet definitive efficacy trials.

So "Emerging" here means: a reasonable, gently-supported idea, resting on a thin but real evidence base — worth offering as a complement, with no promise that it will improve handwriting or fine-motor tasks on its own. The occupational therapist remains the one who can actually help with those.

A note on this method

Jeremy's own synthesis

The four-part structure used across this programme — Opening, Phase I, Phase II, Closing, with the figure watched and then stepped into — is Jeremy's own synthesis, drawn from the Ericksonian and solution-oriented tradition he trained in. It has not been tested as a unified protocol in any clinical trial. The individual ingredients (motor imagery, action observation, breath work) have research behind them, as noted above; the particular way they're combined here is a considered personal design, offered in good faith, not a validated treatment.

Please treat this as a complement to your care, never a replacement for it. Guided imagery sits alongside occupational therapy, medication, and the guidance of your Parkinson's team — it does not stand in for any of them.
